create select box to search different dirs

We use mysql and php for our intranet. Our coding skills are moderate.

We have a request to use a previously installed PhpDig version 1.8.3 to be able to choose from several department directories in a select box (dropdown) to search for pdf documents.

I've plowed through a lot of threads on this subject and the solution is not clear to me. I do see where one can pass the relative "path" variable in a hidden input field. Is this path relative to the form page or to search.php? A complete coding example would be terrific. Thanks in advance. - Mike
