exclude filenames

Hello,
I try to exclude all .php, things like index.php?entry=123 and empty folders in windows, where the field file in spider table in the database is empty or shows something like ?C=N;O=D.
I added "entry" and "?C=" to BANNED and php to FORBIDDEN_EXTENSIONS, but that didn't help.
What did I wrong?
