Well I was just playing with the templates so I did this one...if you want to :rolleyes:

I don't give pictures because the form only support one file attached.

Well he refused it because it was html...so I renamed it .txt, just change that and here you are.

EDIT: attachment removed as requested, see below for another attachment

What the hell : I've upload the personified file not the standard one...
Is it possible for an admin to delete the first file?
